Forge Medical Staffing, LLC (“Forge,” “we,” “us”) provides locum tenens / contract anesthesia staffing and related operations tools for clinicians and facilities.

What this policy covers

This is the privacy policy for the Forge Medical Staffing locum product: the locum site at app.forgemedstaffing.com, the iOS app, and the Android app. It covers the account, schedule, Vault, hours, camera, and messaging information described below.

This policy describes how we collect, use, and protect information in connection with our staffing platform and our SMS / text messaging program used for operational staffing communications (for example: account invites, weekly schedules, and clock-in / clock-out reminders).

iOS and Android apps

The iOS and Android apps are Capacitor clients. They include a native tab bar, show your account in an in-app WebView, and request camera access when you add a Vault credential or a travel receipt.

They are invite-only staffing tools for clinicians. They do not open the admin operations site. Forge is non-clinical. It does not diagnose, treat, cure, or monitor patients. It is not a medical device and it is not medical advice.

Camera and photos

The app asks for camera or photo-library access when you upload a credential to Vault, or a travel receipt on Timesheets when that hospital allows travel expenses. Vault credentials may include licenses or ID photos you choose to send. There are no in-app purchases.

Account deletion

Open Settings → Delete my account and type DELETE to confirm. That closes your login, marks the person inactive, and removes your name, email, and phone from the live record. Hours or pay history Forge must keep for records stays without your contact details.

Facility bill rates

Locums never see facility bill rates. Bill rates and Forge Pay / Money tools stay with Forge staff on the admin operations site.

SMS / text messaging program

The headings that follow are the program disclosures. They sit under this product privacy policy.

Information we collect for messaging

When you opt in to SMS, we may collect and store your mobile phone number; your name and role; messaging consent / opt-in records (time, method, and related disclosures); and message delivery logs needed to operate and troubleshoot the program.

How we use this information

We use mobile numbers and consent data only to send the operational messages you opted into; confirm opt-in, honor STOP / HELP requests, and maintain compliance records; and improve reliability and security of the messaging program.

Non-sharing of mobile information

We do not share, sell, or provide your mobile phone number or messaging consent data to third parties or affiliates for marketing or promotional purposes. We may use service providers that process data solely to deliver our messaging service. Those providers are not permitted to use your mobile number or consent for their own marketing.

Opt-out and help

Data retention

We retain messaging consent and related logs as needed for legal, compliance, and operational purposes, then delete or de-identify when no longer required.

Changes

We may update this policy. The effective date above will change when we do. Continued use of the locum app, the locum site, or the SMS program after an update means you accept the revised policy.
